Dear Students,

To ensure the safe and effective use of the Physical Training Room at Lai King Campus, all full-time students wishing to access this facility must complete a pre-qualification check. According to regulations, applicants must meet one of the following criteria:

1. Completion of the "Briefing on Proper Ways to Use Fitness Equipment" with a passing assessment, organised by Leisure & Cultural Services Department (LCSD); or
2. Completion of the "Fitness (Multi-gym) Training Courses" organised by LCSD from May 2006 onwards, with an attendance rate of 80% or higher.

Please complete the online application form together with the required documents during the period 9 December 2025 – 8 January 2026:

• Receipt or screenshot of your LCSD Fitness Room booking
• Completed Risk Assumption Consent Form
• Completed Physical Activity Readiness Questionnaire

Eligible applicants will receive a confirmation email granting access to the Physical Training Room by 12 January, 2026 (Monday).

Remarks: Those who have already passed the qualification check in the last application need not apply again.

為確保安全及有效使用茘景校園體能訓練室，所有全日制學生如欲使用體能訓練室設施，必須先完成認證前資歷審查。根據規定，申請人須符合以下其中一項條件：

1. 曾參加康樂及文化事務署舉辦的「正確使用健身室設施簡介會」並通過測試；或
2. 曾參加康樂及文化事務署於二零零六年五月或以後舉辦的「器械健體訓練班」，出席率達八成或以上。

請於 2025年12月9日至2026年1月8日 填寫以下網上表格，並提交所需資料：

Ÿ  填妥之承擔風險同意書

Ÿ  填妥之體能活動適應能力問卷
